Drug Characteristics
  • Formulation

    Tablets: 500 mg

    Infusion: 2.5g in 250ml

  • Dose

    Cryptococcal meningitis: 100 mg/kg/day IV in 4 divided doses

    (Should be used in combination with IV amphotericin B)

    Higher dose 150 mg/kg/day when used with fluconazole

  • Side Effects

    Nausea, vomiting, diarrhoea, rashes; less frequently confusion, hallucinations, convulsions, headache, sedation, altered LFTs; blood disorders including thrombocytopenia, leukopenia and aplastic anaemia reported

  • Interactions

    Additive toxicity with other myelosuppressive drugs e.g. ganciclovir, zidovudine, cotrimoxazole

  • Renal

    GFR (ml/min)

    20–40: 50 mg/kg 12–hourly

    10–20: 50 mg/kg 24–hourly

    <10: 50 mg/kg then dose according to levels.

    Alternative dosing from Renal Handbook <10: 0.5–1 g daily is usually adequate

  • Hepatic

    Flucytosine is excreted primarily through the kidneys. No dosage recommendation is available in hepatic dysfunction

  • Pregnancy

    Contraindicated – 1 st Trimester
